Forms for Pensioners

If you are looking to apply for your TSF pension benefit, the following forms are required and must be submitted to the Pensions & Benefits office at least two months prior to your last day of work:


*  If you are not a resident of Prince Edward Island, please choose your province of residence in order to be linked to your appropriate provincial tax form.

NOTE:  When applying for your pension benefit, you must also provide a photocopy of your driver's license or birth certificate as proof of your age.


If you are the surviving spouse of a deceased TSF member, you must complete the following forms and submit them to the Pension Office in order to initiate the spousal pension benefit:

*  If you are not a resident of Prince Edward Island, please choose your province of residence in order to be linked to your appropriate provincial tax form.

NOTE:  When applying for your pension benefit, you must also provide a photocopy of your driver's license or birth certificate as proof of your age.

This authorization is open ended and can only cease with a cancellation made by the member...
